Do you want to make the difference in the lives of youth? Catholic Charites is searching for a Home Coordinator to provide leadership and support in the homes of our clients. You will be the eyes and ears of the home you support and work with your Program Manager to support an on-call rotation, budgeting, shopping, staff support, training, and other projects. We work with clients on community integration and this is a great opportunity to use your creativity to come up with projects and outings for the clients to do in the community. You will provide guidance with activities of daily living, such as money management, personal care, cooking, and cleaning. Our Home Coordinators attend meetings, therapy appointments, doctor's visits, and school meetings with the clients. You will be responsible for supporting the Individual Support Plan at all times and assist clients in reaching their goals.



  • A bachelor or associate degree from an accredited college or university with a major in social work; sociology; special education; psychology; counseling and guidance; criminal justice; nursing, certified nursing assistant, or other health related field; education; or any other area in a human services field.
  • Or current enrollment in and regularly attending an accredited college or university with a major listed above.
  • Or one year of full time work experience working with individuals with severe emotional disturbances and/or developmental disabilities, including autism.
  • Or certification as a child and youth care worker.
  • Must be 21 years or older per state licensing requirements.
  • Must maintain a valid driver's license, appropriate proof of auto insurance, and a satisfactory driving record.
  • Must have the ability to lift 25-50 pounds and tolerate medium to high physical effort including walking, standing, lifting, stooping, squatting, bending, reaching, pushing, and/or pulling.
