Our premier Adult Day Center is hiring a Registered Nurse. In this role, you will be responsible for supervising, training, supporting, and providing leadership for our talented C.N.A. staff. This position is a dream come true - you will work day shift Monday through Friday - no weekends, no mandatory overtime! In this role, you will have the opportunity to provide guidance to Catholic Charities/Hope Haven Program Directors regarding policies, procedures, and other health related needs.


Primary Responsibilities: 

  • Hiring, Training, and supervising C.N.A.'s in compliance with all employment regulations and guidelines.
  • Assesses nursing services on an ongoing basis to evaluate and improve processes and procedures for the most effective and efficient program operations.
  • Provides personal cares and activities of daily living as outlined in the individualized ADC care plan.
  • Understands and practices a client centered approach by being mindful of each participant's abilities, preferences, and needs. 
  • Establishes a trusting relationship with clients and their supports.
  • Monitors service delivery and participates in meetings addressing clients' progress with individual care plans.
  • Maintains, updates, and ensures all health-related documentation complies with DHS, COA, Catholic Charities program standards, and other contracted service agreements. 
  • Conducts TB and health screens for new clients at Hope Haven. 
  • Performs other duties as assigned by the ADC Director. 




  • Graduate of an accredited RN program
  • Current Wisconsin RN license, in good standing with the state licensing board



  • Two or more years of current long-term care experience as a Registered Nurse working with older adults who have physical and/or cognitive disabilities, mental health diagnosis, or dementia/Alzheimer's.
  • Experience supervising and training others.
  • Knowledge of or ability to learn and apply applicable standards of practice to ADC setting.



  • Ability to provide effective leadership, organization, problem solving, and decision-making skills.
  • Effective interpersonal, oral, and written communication skills.
  • Ability to work effectively in a fast pace and changing environment with multiple priorities and objectives.
  • Ability to lift 25 to 50 pounds and tolerate medium to high physical effort including walking, standing, stooping, squatting, bending, reaching, pushing, and /or pulling.
  • Effective and excellent critical thinking and observation skills
  • Ability to understand and follow individual participant care plans.
  • Ability to be flexible and meet the needs of participants and the programs.
  • Competent in providing all aspects of nursing cares.
  • Demonstrated ability to research and inform staff of new or changing protocols when those changes affect client care.
  • Maintaining a valid drivers license, appropriate proof of auto insurance, and a satisfactory driving record are required.
