Hope Haven is expanding its team to include a Medical Case Manager. We are looking for someone with a warm, positive, and supportive presence who can support our clients and advocate for their medical needs. This role is responsible for coordinating and managing services for clients participating our residential program.  Responsible for providing supervision and support of residents in Hope Haven's residential programs.  Our ideal candidate will possess strong communications skills, be able to work as team player and independently, have strong time management skills, and be able thrive under pressure.


Principle Accountabilities


  1. Must provide a welcoming, positive and supportive presence for all clients.
  2. Performs comprehensive and complex case management services which includes the conceptualization, monitoring, evaluation, and facilitation.
  3. Facilitate client education and educational discussion groups as assigned, especially as they relate to health and wellness issues.
  4. Collaborate with primary counselors on all medical and health and wellness issues.
  5. Administers urinalysis, and tracks and records results.
  6. Takes the lead role for oversight of medication-related issues.
  7. Documents notable interactions with clients including observations related to clients' attitudes related to treatment and recovery, interactions with peers, compliance with rules and regulations, and other relevant information.
  8. Assists in monitoring agency compliance with applicable policies, procedures and guidelines.





  • Bachelor's degree in social service field; graduate degree preferred.
  • 2-3+ years' experience in case management.
  • Familiarity with substance use disorders and general recovery principles is preferred.
  • Familiarity with evidence-based practices such as Motivational Interviewing, CBT, mindfulness-based relapse prevention strategies, and others.
  • Strong problem-solving and crisis management skills. Must be able to think quickly on your feet during a high-stress situation.
  • Skilled in active listening and supportive counseling.
  • Clear understanding and boundaries related to personal recovery, 12 Step self-help, and professional treatment.
