The Birth-to-3 Program, a program within Catholic Charities, has an immediate opening for a Physical Therapist in Sauk County. The Birth-to-3 office is in Reedsburg and this position travels to and works in homes located throughout Sauk County. We have a strong emphasis on team approach, coaching and the primary provider model. This is an excellent opportunity to work with children and families in community settings! This is a full time position offering 30 hours per week with a flexible schedule!

Principle Accountabilities

Client Services

  • Travel to clients' homes to conduct therapy.
  • Perform and provide thorough assessments for each child entering the program.
  • Develop and implement an individualized family service plan.
  • Provide support and services to increase the family's capacity to meet the developmental needs of the child.
  • Review and revise written plans as needed in accordance with current objectives.

Program Management

  • Comply with all agency, county, and state paperwork.
  • Work effectively as a member of the multi-disciplinary team.
  • Participate in on-going training and professional development.

Qualifications

  • Graduate of a school of physical therapy and a Wisconsin licensed Physical Therapist are required.
  • Strong evaluation skills.
  • Excellent communication and interpersonal skills.
  • Must maintain a valid driver's license, appropriate proof of auto insurance, and a satisfactory driving record.
  • Ability to lift 25 to 50 pounds and tolerate medium to high physical effort including walking, standing, lifting, stooping, squatting, bending, reaching, pushing and/or pulling.
