Our Adult Day Center located on the east side of Madison is in need of an experienced Licensed Practical Nurse (LPN) to join our team! With support from the Registered Nurse and under the direct supervision of the Program Director, the L.P.N. performs nursing duties within scope of practice. We support around sixty clients (ages 18+) each day and provide them a safe space to be while caregivers are at work. We also provide a variety of recreational activities. The L.P.N. will assist clients with their medical needs and supports our C.N.A. staff as needed throughout the day. This full-time position is 1st shift, Monday-Friday with paid holidays, no mandates, and no weekends required, which is rare to find working in healthcare!

Principle Accountabilities

General Management:

  • Hire, train, and supervise Certified Nursing Assistants in compliance with all employment regulations and guidelines.
  • Assess nursing services on an ongoing basis to evaluate and improve processes and procedures for more effective and efficient program operations.

Client Services:

  • Direct activities of Certified Nursing Assistants in providing services to program participants.
  • Provide personal cares and activities of daily living as outlined in individualized care plan.
  • Assess for changes in participant's condition and behavior and report to appropriate staff and formal supports in a timely and accurate manner.
  • Understand and practice a person-centered approach by considering each participant's abilities, preferences and needs.
  • Establish a trusting relationship with each participant and their supports.
  • Monitor service delivery and contribute at meetings addressing participant's progress with individualized care plans.
  • Perform other duties as assigned by Program Director or delegated by Registered Nurse

Program Management:

  • Provide day to day guidance, support, and supervision to the Certified Nursing Assistants.
  • Support the Program Director in identifying and problem-solving areas requiring additional staff training or mentoring as well as concerns related to participant's physical and psycho-social needs.
  • Ensure participants receive quality of care by utilizing best practice standards in the delivery of daily cares.
  • Maintain, update, and ensure all health-related documentation is in compliance according to DHS, COA, and Catholic Charities Adult Day Center standards.
  • Participate in ongoing training and professional development as outlined in DHS annual continuing education standards and Catholic Charities Adult Day Center program manual.
  • Participate in the Performance & Quality Improvement (PQI) process and assist with the development and implementation of annual program goals providing quarterly updates to the Program Director.
  • Support the Program Director in developing and implementing Adult Day Center policies and procedures.
  • Perform other duties as assigned by Program Director or delegated by Registered Nurse

Qualifications

Education

  • Graduate of an accredited LPN program.
  • Current Wisconsin LPN license, in good standing with the state licensing board.

Experience:

  • 2+ years of current long-term care experience as a Licensed Practical Nurse working with adults who have physical and/or cognitive disabilities, mental health diagnosis, or dementia/Alzheimer's.
  • Experience with safe medication practices including receiving orders, creating Medication Administration Records, passing medications, blood glucose monitoring, and insulin injections.
  • Experience supervising and training Certified Nursing Assistants

Skills:

  • Ability to provide effective leadership, organization, problem solving, and decision-making skills.
  • Effective interpersonal, oral, and written communication skills.
  • Ability to work effectively in a fast pace and changing environment with multiple priorities and objectives.
  • Ability to lift 25 to 50 pounds and tolerate medium to high physical effort including walking, standing, stooping, squatting, bending, reaching, pushing, and /or pulling.
  • Effective and excellent critical thinking and observation skills.
  • Ability to understand and follow individual participant care plans.
  • Knowledge and experience with personal cares, mechanical lifts, and medication administration.
  • Ability to respectfully approach and provide positive redirection for individuals who have various diagnoses.
  • Ability to be flexible and meet the needs of participants and the program.
